We need the New Patient Paperwork turned in
at least one day before your appointment.
If we do not have it by the day before,
we will need to reschedule your appointment.
If you are late to your new patient appointment,
it may result in rescheduling your appointment.
If you no show to your new patient appointment,
you will not be able to reschedule with our office.
